Upon completion of an initial application and interview, the prospective Provider is required to:
1. Contact the local fire department for inspection and approval. (A fire extinguisher and adequate numbers of smoke detectors are required.)
2. The Home Visitor will review the Regional Health Department Checklist with the Provider
3. Consent to a Children's Aid Society check, Security Clearance Request and a Vulnerable Sector Screening with clear results for self and all persons over the age of eighteen (18) years, residing in the home. (Revised 2004)
Every Provider must sign a waiver stating that no one in their family or household has ongoing, or has had in the past, police involvement. Disclosure is mandatory.
4. A family medical form must be completed as required by the Day Nurseries Act. The Provider is also required to have a tuberculosis test with negative results.
5. Providers must make arrangements with a relief person over the age of eighteen years, to take over in case of an emergency. The relief person must complete and sign the Emergency Back Up form.
(Note' Parents must ensure that they too have emergency back-up in case the Provider back-up is not available. The agency will make every attempt to place children in the event the usual Provider is unable to do so, but is not responsible for emergency care.)
6. Providers are required to provide the names and contact phone numbers for three (3) personal references.
